M-S School Registration Requirements for 2020/21
All Pre-K, Kindergarten, 6th and 9th grade students must submit the state required physical forms and immunization records as listed below before they will be allowed to enter school in the Mahomet-Seymour District.
State of Illinois requirements by grade are:
Pre-K:
Birth certificate, must be a certified copy
Current physical dated within one year of first day of school.
Immunization records that reflect the following:
o4 doses Tetanus
o3 doses Polio
o1 dose MMR
o3 doses Hepatitis B
o4 doses Hib titer
o1 dose Varicella (chicken pox) vaccine
oPneumococcal series, or one dose after age 2
Kindergarten:
Birth certificate, must be a certified copy
Current physical dated within one year of first day of school
Immunization records that reflect the following:
o4 or more doses Tetanus – most recent must be dated after 4 years of age
o4 or more doses Polio – most recent must be dated after 4 years of age
o2 doses MMR
o4 doses Hib titer (not required after 5th birthday)
o2 doses Varicella vaccine – first on or after 1st birthday, second no less than 28 days later
oCurrent Dental Examination
oCurrent Vision Examination completed by an eye doctor
6th grade:
Current physical dated within one year of first day of school
Immunizations as per requirements listed above, plus
oProof of having received aTdap booster, a second varicella vaccine and,
oProof of having received one Meningococcal vaccine
Current Dental exam
9th grade:
Current physical within one year of first day of school (must be a complete physical, NOT a sports physical)
Immunizations as per requirements listed above, plus
oProof of having received at least one Tdap booster, a second varicella vaccine and,
oProof of one Meningococcal vaccine
- New for 2019/20 – Current Dental exam
12th grade
- Proof of having received 2 doses of Meningococcal Vaccine w/ the 2nd after age 16
6th thru 12th grades: Proof of having received at least one Tdap booster
2nd, 3rd, 6th thru 12th: Proof of having received two doses of Varicella (chickenpox) vaccine.
6th thru 10th grades: Proof of having received one dose of Meningococcal vaccine
All students who are new to the district in any grade will be required to provide a certified copy of their birth certificate, a current physical, complete immunization records, current vision exam, and proof of residency.
